CONSULATE-GENERAL OF BRAZIL IN VANCOUVER
Due
to the high number of applications received, the Consulate-General does
not confirm the receipt of documents neither checks the status of any
process over the phone or email.
- The Consulate-General in Vancouver issues death certificates for Brazilian citizens who have died in Canada, under its jurisdiction (Alberta, British Columbia, Saskatchewan, Northwest Territories and Yukon).
- To produce legal effect in Brazil, the death certificate must be registered at the Civil Registry Office (Cartório do 1o. Ofício de Registro Civil).
REQUIREMENTS:
- Official death certificate issued by the Canadian Registrar General - original and copy.
- Death Certificate form, dully filled up and signed.
- Cremation certificate, if applicable - original and copy.
- Medical statement indicating the causa mortis - original and copy.
- Original passport from the deceased, for cancellation.
- Valid identification document of the application - original and copy.
FEE:
- The death certificate is free of charge.
- Additional copy of the death certificate: CAD$6.25
FORMS OF PAYMENT:
- Money order or certified cheque only.
- Must be made out to the "Consulate-General of Brazil".
- The Consulate does not accept payments in cash, personal cheque or debit/ credit cards.
APPLICATION BY MAIL:
- The application can be sent by mail. However, the applicant must come in person to pick it up and sign the registry book.
- Once the certificate is ready, the Consulate will contact the applicant to book an appointment.
- Please make sure you have the tracking number for your envelope for your own control.
- Due to the large number of applications received everyday, the Consulate is unable to confirm the arrival of any document.
- The Consulate is not responsible for lost, stolen, damaged or delayed mail.
- Incomplete applications will not be processed and will be return to the sender.
RE-ISSUANCE OF THE DEATH CERTIFICATE:
- A new copy can only be requested if the original certificate was issued by this Consulate and if it have not yet been registered at the Civil Registry Office in Brazil. There will a charge of CAD$6.25 per copy.
